1. Ration Card
2. Light bill or Telephone (MTNL bill only)
3. Society Letter
4. Bank Book or Bank Statement (At least 1 Year old)
5. Voters ID
6. Aadhar card
7. For Renters (L&L Agreement with Owners NOC Affidavit)

2 ] For Birth Proof (Any one) :
1. Birth Certificate
2. School/College Leaving Certificate
3. S.S.C Certificate

Minors or infants passport application following documents required : 
1. Birth Certificate
2. Father and Mothers passport 
3. Any two address proof of Father or Mother(given above)
4. Annexure 'H' 
5. Minors affidavit on stamp paper

If one parent is resident abroad, a Sworn affidavit by the parent resident abroad attested by the Indian Mission along with affidavit from parent residing in India as well be submitted

Note:- Those who born after 1989, Birth Certificate is compulsory.
